For your first WIC appointment, you will need to bring identification for you and your child(ren), proof of residency and income, and your child’s immunization record.

Bringing these documents will help make your first WIC appointment go smoothly.

- New Baby ID- Hospital discharge papers or hospital ID bracelet
- Driver’s License, School, State or Military ID
- Passport
- Voter Registration Card
- Birth Certificate
- Shot Record
- Health Insurance/Medicaid Card
- Social Security Card
- Social Services Benefit Letter

- Utility OR Personal Bill mailed to residence in the last 30 days
- Rent OR Mortgage Receipt
- Letter From A Reliable 3rd Party (church, social services, employer, shelter)
- Pay Stub issued within the last 30 days

- Current Pay Stub Within 30 Days
- W-2 OR Income Tax Return
- Signed Statement by Employer (statement must be printed on employer letterhead, include pay rate and frequency, and be signed and dated by employer)
- Appropriate Documentation for all other income sources (SSI, child support, unemployment, etc.)
