Access to WIC approved infant formulas and nutritionals, as well as the most up-to-date Medical Documentation Form (also known as a Special Formula Form or WIC-27).
Formulas Offered by WIC
WIC offers a variety of standard Similac formulas, as well as specialty formulas for infants with specific medical needs. A Medical Documentation Form (also known as a Special Formula Form or WIC-27) is required for specialty formulas.
The infant formulas listed below are WIC approved standard formulas and only require a WIC-27 if they are prepared at a dilution different from what is stated on the product label:
- Similac Advance
- Similac Sensitive
- Similac Total Comfort
- Similac Isomil
A WIC-27 is required when requesting a non-contract standard infant formula, exempt infant formula, or WIC-eligible nutritional. Please note that a prescription from a health care provider cannot be accepted in place of the WIC-27, and approval requires a qualifying medical condition. Formulas not on the Missouri WIC Approved Formulas list can not be issued even with a WIC-27 or Doctor’s note.
