Birth and Death Certificates
Read the full list of what documents are needed to receive vital records from either of our locations.
Certified copies of birth certificates for a person born in the state of Missouri after 1920 or for a death in the state after 1980 are available at both TCHD offices.
Please note, in order to receive a vital record, a photo ID or two alternate forms of identification are required.
Alternate forms of ID can include:
- Cancelled duplicate check (must show name, address, signature, and name of institution)
- Certificate of vehicle title or registration documents
- Court certified adoption papers that include adopted parent(s) name
- Insurance policy (health, home, life, etc.)
- Letter from government or social agencies
- Medicaid/Medicare document or identification card
- Military discharge document (DD-214)
- Official certified deeds or title to property
- Payroll stub that includes the Social Security number of the applicant (cannot be handwritten stubs)
- Proof of auto insurance
- School yearbook
- Shelter name band (including name of shelter)
- Social Security card or Social Security numident printout (print out of an applicant’s Social Security account of activities)
- Utility bills that show name and address of applicant (water, gas, electric, telephone, etc.)
- W-2 form issued within the last year in addition to a signed Social Security card (social security numbers must match)

For birth certificates prior to 1920 or death certificates prior to 1980, you must contact Bureau of Vital Records at the Missouri Department of Health and Senior Services.

Out of state certificates must be obtained from the appropriate state vital records department. To find the appropriate department, visit the CDC’s National Center for Health Statistics, or contact the Taney County Health Department.
