We offer immunization services for all ages as part of our preventive care and wellness mission.
Why Immunize?
- Vaccines work with your body’s natural defenses to build protection and reduce the risk of serious illness.
- By getting immunized, you lower your risk and help protect people who cannot receive vaccines (infants, those with weakened immunity, etc.).
- Routine immunizations are a key part of keeping your family, your workplace, and our community safe and strong.
Immunization Services Include
TCHD’s clinics are staffed by registered nurses who provide education, administer vaccines, and maintain immunization records.
Immunization services include:
- Routine childhood vaccinations
- Adult immunizations
- International travel vaccines
Children through age 18 years are eligible to receive vaccines at no charge through the Vaccines for Children (VFC) program if any of the following apply:
- Uninsured
- Medicaid eligible
- Native American Indian
- Native Alaskan
- Or child’s insurance policy does not cover vaccines
Infants, children, and adolescents must be accompanied by a parent or guardian to receive immunizations.
TCHD accepts most health insurance plans, as well as Medicaid and Medicare, for immunization services. Coverage can vary among different insurance plans. It is always advisable to check with your insurance provider for coverage specifics before receiving immunizations, as you may be responsible for charges that are not covered by your insurance policy.
Services are provided by appointment. Please bring your insurance card, current immunization records, and a photo ID.

Immunization Exemption Forms
Actively choosing not to immunize a child is a parent’s right; however, it carries significant responsibility. To protect inadequately vaccinated individuals and the entire community, unimmunized children could be excluded from school during disease outbreaks. Exclusion from school or daycare can cause a hardship for the child and parent, however no exceptions are made, regardless of the circumstances.
Parent/Guardian Exemption
An exemption represents a parent or guardian’s objection to immunizing their child/children.
Religious Exemption
Religious exemption represents a parent or guardian’s stance that their family’s religious beliefs do not support immunization.
